$160 per 50-minute session.
Currently I am a provider with First Choice, Group Health Access PPO (now called Kaiser Permanente), Premera and Regence Insurance and with the following Employee Assistance programs: HMS (A Health Advocate Company), First Choice, and Wellspring. Many health plans allow for out-of-network coverage and I am considered an out-of-network provider for most insurance companies. Please check with your insurance carrier to see if they will allow out-of-network services. If they do, I can provide you with a statement that you can present to your health insurance company. Please be advised, however, that payment is due at the time of service, and that by submitting your bill, your insurance company will reimburse you directly at their stated rates. Services may be covered in full or in part by your health insurance or employee benefit plan.
I’d recommend asking these questions to your insurance provider to help determine your benefits:
• Does my health insurance plan include mental health benefits?
• Do I have a deductible? If so, what is it and have I met it yet?
• Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
• Do I need written approval from my primary care physician in order for services to be covered?
I accept cash, check and all major credit cards as forms of payment.
If you are unable to attend a session, please make sure you cancel at least 48 hours beforehand. Otherwise, you may be charged for the full rate of the session.
